The diamond plated floor material is porus and in a state of disrepair. Repair damaged wall across from the hand sink near the back door in the basement. Wall has a hole in it and the wall material is falling apart. In the kitchen discontinue using cardboard as floor matting in the kitchen. Clean the hood and all attachments to the hood above the cooking line. Remove grease buildup. Clean stairs leading up to the kitchen from the basement. Clean the floor areas under the cooking line. Roche observed moving near kitchen area. Provide extermination service and provide a reciept of service at the compliance inspection. Alos provide a status report from exterminator and provide a game plan from the exterminator that details methods used to address roche issue.
