Dishmachine behind the bar and the dishmachine in the warewash area are still not working. The PIC was able to call the person that they use and he stated he would be out next Thursday to fix both machines he was waiting on parts. Observed the AC unit over the dishmachine and over the back stairway is still dripping water. Repair Observed the freezer door still not closing properly and observed the 2 fans inside the freezer not working causing heavy frost build up. Observed water damage in the ceiling of the basement. Contact a licensed professionals to come address these problems ensure permits are pulled sign off on and approved.

Controlling Pests (Pf)
PIC was able to provide me with the pest report from 9/3/25. Report states heavy rat activity. Observed a heavy amount of rat activity both basements and under the bar area. Observed flies heading down into the basement and in the warewash area. The AC unit is located over the dishmachine and over the stairway to the basement is leaking water. Provide a detailed IPM report from the license exterminator. Report needs to be free and clean of rodent activity. Area needs to be cleaned and sanitized. Recommend having the exterminator open the booths in the dining area to place traps. Exterminator will need to come out more frequently until the problem has been resolved and there is no more rodent activity.

SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.

Remove dust from the vent in the mens room. Clean to remove. Clean and organize basement. Ensure all items are 6 inches off the floor and repair all holes in the wall and ceiling.
