Observed the AC unit over the dishmachine and over the back stairway has been shut off and still not repaired. Observed the freezer door still not closing properly and observed the 2 fans inside the freezer not working causing heavy frost build up. Still not repaired. Observed a running toliet in the bathroom. Repair toliet. Observed leaky faucet in the 3 bay sink. Repair faucet Contact a licensed professionals to address all the repairs and provide an invoice on company letterhead.

Controlling Pests (Pf)
PIC was able to provide an updated pest report from earlier today 9/11/25. Report report states no activity. At today's re inspection 9/11/25 rat dropping were observed in the basement. Observed flies in the basement and in the restaurant and fruit flies under the sink behind the bar and in the ware wash area. Observed open whiskey bottle at the time of the inspection. Ensure staff is trained on wiping down bottles and putting covers on the bottles before closings. Provide a detailed IPM report from the license exterminator. Report needs to be free and clean of rodent activity. Ensure exterminator is also treating for flies and fruit flies. Recommend more frequent inspections until the problem has been resolved and there is no more rodent activity. Provide reports to ISD Health.
