Dishmachine behind the bar and the dishmachine in the warewash area in not working. Repair Observed the HVAC unit over the dishmachine and over the back stairway is leaking. Repair Observed the freezer door not closing properly and observed the 2 fans inside the freezer not working causing heavy frost build up. Repair Contact a licensed professionals to address all the repairs and provide an invoice on company letterhead.
Why This Matters
This violation directly contributes to foodborne illness risk
Code Requirements
Immediate correction required. Follow health code section 105 CMR 590.005. Implement proper food safety procedures. Train all staff. Document corrective actions taken.

Controlling Pests (Pf)
Owner was able to provide a detailed IPM report from 9/19/25. Report states heavy roach activity in the kitchen and 5 mice catches. Observed drain flies in the bar area. Observed liquor bottles behind the bar that still have the pourers attached. Spoke to the PIC and recommended removing all the pourers at night ensure to clean/sanitize the pourers wipe down all the bottles ensure all bottles have a cap and ensure that the soda gun is cleaned or soaked at night to remove sugar ensure the mirror and shelving are cleaned and sanitized. Provide a detailed IPM report from the license exterminator. Report needs to be free and clean of rodent activity. Ensure exterminator is also treating for drain flies. Recommend more frequent inspections until the problem has been resolved and there is no more rodent activity. Provide reports to ISD Health.
Why This Matters
SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.

IMMEDIATE ACTION: Clean/sanitize all affected areas with 10% bleach solution. Discard ALL exposed food. Seal holes over 1/4 inch (mice) or 1/2 inch (rats). Remove food sources. Install bait stations. Professional extermination within 24 hours.

Outer Openings Protected (C)
One of the exteriors doors in the the back near the landlords basement needs to reattach the weather stripping and another door sweep on back of the door ensure no daylight is visible. 2nd exterior door near the prep area has foam stray all around the edge. Provide proper door sweep or repair door to ensure no day light is visible.
Why This Matters
PEST INVASION: Open doors allow entry of flies (carry 100+ pathogens), rodents (35+ diseases), and airborne contaminants. One fly can contaminate entire prep area in minutes, landing on 30+ surfaces spreading E. coli, Salmonella.
Code Requirements
Install self-closing devices on ALL exterior doors immediately. Doors must close completely within 3 seconds. No gaps over 1/4 inch. Consider air curtains for frequently used doors. Keep doors closed when not in active use.
