No vomit and diarrhea kit on site. Provide vomit and diarrhea kit. The vomit and diarrhea procedures and supplies document will be emailed over with this report. Please print out and have it stored with the kit. Spoke to the PIC and Manager and they stated it was ordered but they were not able to provide documention or work order.

This violation supports conditions that may lead to foodborne illness

Immediate correction required. Follow health code section 105 CMR 590.002. Implement proper cleaning & sanitization procedures. Train all staff. Document corrective actions taken.

Controlling Pests (Pf)
Evidence of rodent droppings near the media cage in the back room. Clean to remove droppings and contact licensed exterminator to conduct an inspection. Provide pest report at the re inspection. The rodent droppings have been cleaned up near the media cage. Evidence of droppings on the wall near the back door. The last extermination report was on 3/12/25. The last inspection report stated to contact the licensed exterminator. On the last 2 reports it states no evidence of droppings but there is evidence of droppings.
Why This Matters
SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.
Code Requirements
IMMEDIATE ACTION: Clean/sanitize all affected areas with 10% bleach solution. Discard ALL exposed food. Seal holes over 1/4 inch (mice) or 1/2 inch (rats). Remove food sources. Install bait stations. Professional extermination within 24 hours.

Observed soil and debris throughout the entire store. Vacuum entire store to remove debris on the carpet. Sweep clean organize the back room to remove debris and soil. This will help with rodent activity and also help the exterminator.

Observed debris in and around the trash area. Also in between the trash and back platform. Clean and remove trash and debris outside. 4/10/25- They cleaned between the trash and back platform but did not clean in and around the trash area. PIC stated that the trash compactor was broken and that they were getting it repaired. They could not go inside and clean around the trash area because it was to dangerous.
