Observed 2 light bulbs out over the cook top stove. PIC stated that the electrical needs to be fixed. Contact licensed electrician and provide an invoice on company letterhead. 9/17/24- At the time of the re inspection the light bulbs and light switch was repaired and replaced. PIC stated the electrican would be out later today to fix a light in the basement. Please provide an invoice on company letterhead from the licensed electrician.

Provide a licensed trash removal company and provide proper waste receptacles for outside storage. All commerical trash must be removed by a licensed company. On 9/10/25 ISD enviromental sent an email to the establishment. Please update your 2024 site cleanliness certificate. At time of the inspection observed a non working refrigerator stored in the basement in the middle of the floor. PIC stated someone would be by to remove unit. Remove or repair refrigeration unit.
