At the time of the re inspection the rodent droppings have been removed and there is no evidence of drain flies. PIC stated that a licensed exterminator would be out this week. Provide a detailed IPM report from a licensed exterminator at the next inspection. 9/24/24- Owner was able to provide an invoice from a licensed exterminator. The company came out to give a quote and is due back 9/25/24 to complete the full inspection. Please provide a detailed IPM report from the licensed company on company letterhead.

SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.

IMMEDIATE ACTION: Clean/sanitize all affected areas with 10% bleach solution. Discard ALL exposed food. Seal holes over 1/4 inch (mice) or 1/2 inch (rats). Remove food sources. Install bait stations. Professional extermination within 24 hours.

Maintaining Premises Unnecessary Items and Litter (C)

Provide a licensed trash removal company and provide proper waste receptacles for outside storage. All commerical trash must be removed by a licensed company. On 9/10/25 ISD enviromental sent an email to the establishment. Please update your 2024 site cleanliness certificate. At time of the inspection observed a non working refrigerator stored in the basement in the middle of the floor. PIC stated someone would be by to remove unit. Remove or repair refrigeration unit. 9/24/24- Owner was able to provide a quote from the trash company. At the time of re inspection the owner reached out to Republic and they stated that someone would be intouch within 48 hours to come out to the establishment. Recommended purchasing 1 or 2 receptacles. Observed refrigeration unit outside in the trash area PIC stated someone was coming to remove it. Remove broken refrigeration unit. Owner was able to provide me with the site cleanliness receipt 9/23/24. Please ensure all trash and any unnessary items are removed for the next inspection. 9/27/24- PIC called Republic at the time of the re inspection. The dumpster was scheduled to be delivered on Wednesday 9/25 and company canceled. They rescheduled for today 9/27/24 and canceled. Representative stated the dumpster is scheduled for 10/1/24 and she stated that a confimation email would be sent over to myself and owner today.
