PIC did have two pest control reports available at this time. PIC will need to address the issue found by Ecolab. Some of the repairs have been done but not taken off the report. A deep cleaning of the location and repairs in storage should be addressed. PIC should have pest control take a look in the ceiling of the location.

This violation supports conditions that may lead to foodborne illness

Controlling Pests (Pf)
Evidence of mice droppings around the chip shelves. PIC has the pest control inspecting each week. Multiple glue boards under shelves. The PIC will need to clean the floor and put the traps down. Please have pest control address this and with the help of you clean the floor and shelves each day. 9-11-24 PIC will need to review the report from pest control and clean areas patch holes and replace door sweeps as noted in report. 10-15-24 On this visit no new pest control reports at location. Droppings found on shelf in chip area. PIC will need to check each day clean and remove. Pest control will need to do more inspections and any areas that need to have repairs that are noted on the report should be taken care of. 10-23-24 Evidence of mice in chip area and behind chips on a non food shelf. Mutiple droppings found in back storage room along wall in eletrical room and around compacter.We found a live mouse on a grue trap in back room. PIC will need to pull all pallets off the wall so that pest control an properly bait.
Why This Matters
SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.
Code Requirements
IMMEDIATE ACTION: Clean/sanitize all affected areas with 10% bleach solution. Discard ALL exposed food. Seal holes over 1/4 inch (mice) or 1/2 inch (rats). Remove food sources. Install bait stations. Professional extermination within 24 hours.

PIC will need to review the last pest control report and make repairs as noted in the report. All areas will need to be addressed. Door sweep holes in walls and general cleaning of the location as put in the report. 9-30-24 Please continue to make repairs noted by Pest control. If building owner is not cleaning up the barrels you may want to clean out the one in front of our door. 10-15-24 Rodent dropping found on retail shelves in chip area. Retail shelves will need to be cleaned. PIC stated that the barrel at the front door can not be moved. Trash and debris under the barrel will need to be cleaned up. 10-23-24 PIC will need to clean of the retail shelves we found with some droppings on them. The back room had boxes stacked up with some dropping on top of them.

Storage Refuse Recyclables and Returnable (C)
The building has barrels out side along the front of the building. The bags are empty but trash and food is built up under bags. Please clean and or remove barrels until problem is resolved. PIC stated that the owner of the building has been informed of the pest issues. 10-15-24 Barrel and the same debris and trash under the barrel. Please clean or removed barrel. 10-23-24 Barrels are still out side with loose trash under them. Rodent Control from the City was able to get the owner of the building to remove the barrels around the building.

As discussed PIC will need to clean the floor in the back room of rodent droppings. Pallet will need to be stored off the wall so that pest control can do a proper inspection.

PIC will need to clean up the back room. I found shelves open boxes old equipment just in corners. This would be location for mice to live.
