Controlling Pests (Pf)
Large amount of rodent-like droppings at the shelves under snack/food storage area (retail space) and througout the establishment floors. Last Ecolab (pest company) on 05/04/22 IPM states that several patching and repair/construction work must be performed and several cleaning areas pointed out. Follow up with all of them and provide evidence of work being scheduled/performed as described. Unclear at this point how often Ecolab treats the establishment. Provide more frequent service ( possibly weekly).
Why This Matters
SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.
Code Requirements
IMMEDIATE ACTION: Clean/sanitize all affected areas with 10% bleach solution. Discard ALL exposed food. Seal holes over 1/4 inch (mice) or 1/2 inch (rats). Remove food sources. Install bait stations. Professional extermination within 24 hours.

PIC states that they currently have no dumpster and a new dumpster will be received in 7-10 days. Only one large trash barrell at this time but already filled with no additonal place to store trash. Provide more containers/barrels for the trash storage. No Site Cleanliness license posted. Provide a current Site Cleanliness License by ISD-Enviromental Services.

Code Requirements
Immediate correction required. Follow health code section 105 CMR 590.005. Implement proper food storage procedures. Train all staff. Document corrective actions taken.

At this time the establishment is being inspected/treated every three days by ECOLAB. According to the last IPM report the establishment have made great improvement in cleanliness. Work has begin and will continue to address some stuctural issues in the building regarding rodents point of entrance/harborage. Performed all required work.
