Evidence of mice droppings in the back room near the back door on the floor near the break area next to the mop sink on the bottom shelving under the chips on the shelving near the cat litter at the front of the store near the office door. Photos were taken When I pulled back the card shelving I could smell a strong ordor and observed mice droppings. PIC stated that the pest company does not put traps inside the card shelving. This needs to be addressed. Photos were taken. After reading 3 recent pest reports- 10 mice were caught they found a nest under one of the registers. Exterminator states on the reports that cleaning needs to take place every night he needs access to be able to place traps properly traps have been removed at the time of this inspection I pointed out to the PIC that the traps needed to be placed up against the wall I only saw a few traps in the back room. Last report was 5/13/25 5/7/25 4/4/25. CLEAN TO REMOVE ALL DROPPINGS. Contact exterminator to come out and set up more frequent inspections until you receive a clean report.
Why This Matters
SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.

IMMEDIATE ACTION: Clean/sanitize all affected areas with 10% bleach solution. Discard ALL exposed food. Seal holes over 1/4 inch (mice) or 1/2 inch (rats). Remove food sources. Install bait stations. Professional extermination within 24 hours.

Voluntary closure isuued. Provide an updated IPM report from the licensed exterminator. Ensure all violations on this report and the extermination report have been corrected before contacting ISD HEALTH.

Observed boxes being stored on the floor in the back room. This issue has been addressed a few times. The boxes need to be properly stored off the floor to help with rodent activity. Items need to be stored 18 inches from the wall in the back room this is important so that the exterminator can place the traps. Also stated in his report

Observed soiled floors and shelving through out the store. Spoke to the PIC about sanitizing the shelving before placing the items back on the shelves. Observed food items and other items under the shelving throughout the store. Clean under shelving to remove all items. Floors and shelving need to be swept and sanitized to remove all soil and debris.
