PIC Performing Duties
Imminent health hazard 8-404.11. Non potable water has flooded the establishment. Broken glass non potable water on floor. Establishment was operating upon arrival. TSOP per phone conversation with JM. Permit is suspended. Establisment must wash rinse santized all affected areas and provide invoice from proffesional cleaning company to health division. Establishment must change DBA on license to match sign on front of business at 1010 Massachusetts Avenue 4th Floor. Determine source of water and make all necessary repairs.

EXTREME RISK: Without active management, ALL five CDC risk factors go unchecked. Studies show restaurants without certified managers have 2.5 times more critical violations.
