Report to be sent to ISDHealth@Boston.gov A smoke test was done on 9.11.25 smoke was observed coming out from the grease trap and through the 3 bay sink drains Manager was instructed to have a licensed plumber assess the issue & what repairs will be needed The report should be on licensed plumbers letterhead and forwarded to ISDhealth@Boston.gov for review

Evidence of cockroaches and rodent activity under equipment in back hall and in room located above the establishment There are numerous inaccessible areas in the establishment that need to be cleared for full pest control treatment Establishment shall provide updated pest control reports from a licensed pest control company Pest control reports shall indicate any pest activity in the area any structural voids that shall be repaired and any cleaning issues that shall be corrected All reports shall be provided to the Health Department Evidence of pest activity is noted throughout the food court including in seating area and ledges along windows and walls

SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.
