Evidence of cockroach activity under wait station and near clean plate storage. Ecolab had arranged for fogging of building due to "infestation" as stated on pest report dated on 9/7/23 but was unable to do so due to lack of coordination and building being occupied by cleaners. Ensure this treatment is performed within 72 hours. Provide updated IPM pest report upon reinspection. Clean and sanitize all affected areas.
Why This Matters
SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.

IMMEDIATE ACTION: Clean/sanitize all affected areas with 10% bleach solution. Discard ALL exposed food. Seal holes over 1/4 inch (mice) or 1/2 inch (rats). Remove food sources. Install bait stations. Professional extermination within 24 hours.

Numerous dead cockroaches on shelving storing plates in kitchen under wait station side bar "the pass" "industry alley" portion of kitchen . Clean and sanitize all affected areas. Wash/rinse/sanitize/air dry all wares stored in the area. Corrected on site.
