Observed hose coming out of the brick wall near the trash area. Water is draining into the mop bucket. PIC stated the hose was coming from the AC unit. PIC stated that there was a water leak in the basement. No evidence of water at the time of the inspection. Observed leak under the 3 bays sink. Repair. Contact licensed plumber ensure to pull a plumbing permit at 1010 Mass ave for the repairs. Licensed plumber will need to contact the plumbing inspector after the work has been completed and then you will need to provide the signed off permit. Observed gang box (light switch) not installed properly in the womens bathroom. Repair Observed a hole in the floor in the women's bathroom. Repair/ Cap.

Observed 2 refrigeration units that are not commerical grade refrigeration units. PIC stated the refrigeration unit downstairs is not working. Provide commerical grade refrigeration units. Email over the spec sheets for approval.

PEST INVASION: Open doors allow entry of flies (carry 100+ pathogens), rodents (35+ diseases), and airborne contaminants. One fly can contaminate entire prep area in minutes, landing on 30+ surfaces spreading E. coli, Salmonella.
Code Requirements
Install self-closing devices on ALL exterior doors immediately. Doors must close completely within 3 seconds. No gaps over 1/4 inch. Consider air curtains for frequently used doors. Keep doors closed when not in active use.

No evidence of rodents or fly's at this time. Exterminator was here on 8/19/25. Spoke to the pest company on the phone and they are sending over the report by the end of the day today. Please email over the report by the end of the day today.
Why This Matters
SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.

IMMEDIATE ACTION: Clean/sanitize all affected areas with 10% bleach solution. Discard ALL exposed food. Seal holes over 1/4 inch (mice) or 1/2 inch (rats). Remove food sources. Install bait stations. Professional extermination within 24 hours.
