Controlling Pests (Pf)
PIC will need to address the fruit fly issue in the kitchen and warewashing areas. PIC stated that someone has called to address it. Please provide that report to me by email.
Why This Matters
SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.
Code Requirements
IMMEDIATE ACTION: Clean/sanitize all affected areas with 10% bleach solution. Discard ALL exposed food. Seal holes over 1/4 inch (mice) or 1/2 inch (rats). Remove food sources. Install bait stations. Professional extermination within 24 hours.

Repairing-Premises Structures Attachments and Fixtures-Methods (C)
Low temperature dishwasher in kitchen tested at 50ppm but two sets of gauges on machine. Wash looks like it was stuck at 120F and the other stated it was 167F. Please have gauges serviced. 10-8-24 At this time dishwasher has low temp sanitizer hooked up to the machine. The rinse temp was between 120 and 140F and the wash was 187F. We put the dish in the machine and it was 167F at the plate. PLease have one set of gauges that will give the temperature of the machine. Sanitize container was replaced at this time. Not sure it going to be needed it a high temp. Also please have a plumber repair the leak in the wash bay and the fauset at the three bay sink. 10-10-24 PIC will need to have repairs made to the three bay sink and fauset. The dishwasher was addressed and is now showing 50ppm.

Immediate correction required. Follow health code section 105 CMR 590.006. Implement proper food safety procedures. Train all staff. Document corrective actions taken.
