Controlling Pests (Pf)
Observed a dead mouse in the basement. Evidence of droppings in the basement. PIC provided me an pest report dated 7/12/24. He stated they come out once a month. Please remove dead mouse in the basement and clean and remove droppings. Contact licensed exterminator and provide a detailed IPM report for the re inspection.
Why This Matters
SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.
Code Requirements
IMMEDIATE ACTION: Clean/sanitize all affected areas with 10% bleach solution. Discard ALL exposed food. Seal holes over 1/4 inch (mice) or 1/2 inch (rats). Remove food sources. Install bait stations. Professional extermination within 24 hours.

Cleaning Ventilation Systems Nuisance and Discharge Prohibition (C)
Hood sticker expired in April. PIC contacted Boston Hoods at the time of the inspection. They stated they would be out tomorrow night 7/31/24 for the inspection. Please take a photo of the updated hood sticker and send this over to ISDHEALTH@BOSTON.GOV after the hood has been inspected.
