Discontinue the processs of breaking down 20 - 25 pound bags of dry rice into individual smaller bags to sell to the public - No labels or ingredient list is avaialble - nor is there a proper clean area where this procedure can be conducted - Discontinue practices - the store is catagorized as a " retail store only"

Owner has been sited in the past with repeat violations for ongoing rodent problem that evidently has not been properly addressed at this time- pictures taken inside the store of rodent waste scattered throughout the floors of the store especially underneath the chip racks around the large bags of dry rice and inside the compressors of the multi -door refrigeration / freezer units

An abundance of rodent waste is visble throughout the store - rodent droppings are visible underneath the grocery shelves - around the compressors inside the refrigeration / freezer units and around the large bags of dried rice avaialble for retail sale

All previous glue boards / traps and droppings have been removed and replaces with new ones - behind the small blue lottery desk located by the front door appear to be some droppings still visible - No current pest control reports on site either - IPM reports from your professionally licensed pest control company need to be available on site - this will be written statements from the company stating where all the problematic areas for rodent activity are located as well as the corrective actions that have been taken place and what te company is using to address them

Ensure all foods being sold are elevated off the floors a minimum of six inches to reduce rodent activity and other potential contamination

Resurface all wooden shelving units used to store bags of rice and other bulk fod items on the floors of the store as discussed to remove all visible stains soils

Clean all shelving units in the grocery aisles - clean inside and outside of all freezer / refrigeration units -clean all other food storage surface areas where needed from visible food spills stains and visible rodent droppings - Clean the exterior of all the floor compressors underneath the multi door refrigeration / freezer units from rodent droppings and visible soils

Clean floors in the corners of the store especially behind the multi door coolers plastic lottery desk and any other section of the store where visible soils still reside - most of the floors have been properly cleaned and sanitized

Clean floors throughout the store including the stairs leading to the basement from all visible food spills stains soils and rodent droppings

there is a section of the ceiling inside the store that appears to have had a water leak - it has visible water damaged in the area - address and repair as discussed

Several glue boards underneath the chip racks and elsewhere throughout the store are covered with dead insects - clean to remove as discussed

Maintaining Premises Unnecessary Items and Litter (C)

Although much of the loose debris not affiliated with the store have been removed and discarded that was in the rear corner of the store there still remains some clutter and debris - discard all items [ paint cans bag of cement - concrete blocks ] not affiliated with the store and / or organize the remaining items as discussed

Clean to remove all clutter and debris gathering in the back / rear corner of the store - remove all items not affiliated with the store - maintain clean and tidy as discussed

Drying Mops (C)

All in use brooms and mops used to clean the floor of the store shall be elevated off the floors a minimum of six inches off the floors after being used -cleaning solution inside the mop bucket shall be emptied throughout the day and then changed as needed then stored in the segragated area of the store as we discussed - sides of the mop sink do not have barriers on either side - if foods available for retail sale are to be sold on the shelving units ocated near to the area - barriers will need to be provided as we discussed during the interview

Soiled mops and brooms lying on the floor of the grocery aisle - discontinue and store properly [ off the floors and inside the designated mop sink ] to reduce rodent activity
