Heavy presence of small black flies noted in the establishment in the front service area the kitchen prep area and in the warewashing area. Small black flies on products and in packaging of raw products. A completed updated detailed Pest Control Report shall be provided to the Health Department per the hearing decision for the establishment

Per an interview with the PIC an additional employee took the CFPM course and test but did not obtain a certificate.
