Kitchen floor grout observed worn/missing.Re-grout kitchen floor as needed. Some tiles on the dining room floor are loose from what appears to be water damage from under them.Repair PIC states he will submit a time line for repair. Upon re-inspection on 11.9.21 The manager states that the plumbers will be on site to evaluate another area of concern PIC to forward report to the Health division. Upon re-inspection on 11.16.21 The PIC states that there a 2 areas of concern where the plan is to remove the floor tiles address any residual water issues have plumber verify there are no further leaks. Spoke with Principal Health Inspector R.Murad.An extension has been provided. Upon re-inspection on 12.3.21 The PIC is stating and has provided a work order that an additional water leak was located and repaired. An extension has been provided to replace several missing floor tiles in 2 locations. Upon re-inspection on 12.17.21 The PIC stated & provided documentation that the repair service rescheduled the service tech to be on site on 12.17.21.Manager instructed to forward tile replacement schedule when received. Service tech arrived during the re-inspection.
