Evidence of rodent droppings under the chip display/and behind the small refrigerators in front of the store under the bread shelving at the end of Aisle 5 and in the back room. Move the floor displays remove kick boards sweep cleaning/sanitize the floors and shelvings to remove all new/old rodent droppings. All food on these shelves need to be placed in totes over night. Remove any food items that have been damaged. The back room- everything needs to be pulled away from the wall clean/sweep/sanitize the floors to remove debris urine old/new droppings. All items need to be off the floor at least 6 inches. If you have any food items they need to be put into totes. The exterminator removed one catchat the time of this inspection. The PIC will email over today's extermination report after the inspection. PIC was able to provide IPM reports for 12/31/24 and 1/2/25. Burgess Pest Management John was present today during the inspection. Recommend having an exterminator come out every day until this problem has been corrected. At the next inspection be able to provide a clean extermination report. Pest Reports will be uploaded and attached to the the permit number.
Why This Matters
SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.

IMMEDIATE ACTION: Clean/sanitize all affected areas with 10% bleach solution. Discard ALL exposed food. Seal holes over 1/4 inch (mice) or 1/2 inch (rats). Remove food sources. Install bait stations. Professional extermination within 24 hours.

Observed items and debris in the room before entering the walk in refrigeration unit. Sweep/clean floors to remove debris and items. Observed debris and rodent droppings under the chip display in the front of the store. Sweep/clean floors to remove debris and rodent droppings. Observed debris on the floor in the back room. Sweep/clean floors to remove debris and old/new rodent droppings.
