Hood ventilation system at the cook line is not operating. Fire alarm was activated no evidence of fire observed. Establishment Voluntarily closed. BFD inspector Lek Moy was on site prior to my arrival I spoke with the BFD inspector by telephone Establishment to remain Voluntarily closed per BFD inspector BFD inspector states that the hood system(motor/switch) needs to be repaired proper documentation provided before approval to be re-inspected for re-opening.

Hood ventilation system motor is not operating.Repair BFD inspector instructed the manager to provide all necessary repair documents on company letterhead before granting approval to be re-inspected by the Health Department Upon re-inspection A temporary switch has been provided hood system is operating. BFD inspector was contacted and he provided approval to re-open An extension has been provided to complete the repair
