PIC Performing Duties
8-303.20 Existing Establishments Permit Renewal and Change of Ownership. [590.008(C)](A) Copies of Permit. The permit shall be made out in duplicate. One copy shall be given to the applicant and a paper or electronic copy shall be placed on file with the board of health.(B) Expiration and Renewal of Permit.(1) A permit shall expire no later than one year from the date issued(2) An annual food establishment permit may be renewed by applying at least 30 days prior to the expiration of the permit on a form provided by the FC-regulatory authority. Establishment must pay for 2019 Health Permit @ 1010 Massachusetts Ave 4th Floor Health Division ASAP
Why This Matters
EXTREME RISK: Without active management, ALL five CDC risk factors go unchecked. Studies show restaurants without certified managers have 2.5 times more critical violations. Lack of supervision leads to: temperature abuse (bacteria double every 20 minutes), cross-contamination spreading pathogens, sick employees working with food, improper cooking allowing pathogen survival. This single violation enables conditions for major outbreaks.

PIC MUST: Be present during ALL operating hours; Hold valid food safety certification; Actively monitor employee health and hygiene; Ensure proper cooking, cooling, and holding temperatures; Verify foods from approved sources; Monitor handwashing compliance; Ensure equipment properly sanitized; Take immediate corrective actions for violations; Demonstrate knowledge to health inspector; Train and supervise all food employees.

Ductless frialator in back concessions area without access to handsink and kept under an open ceiling. Remove/reclocate or provide closed ceiling and handwash facilities as discussed. Discussed location of handsink in back area. PIC to install handsink in close proximity to self vented frialator approved by Massport FD.
