Conformance w/ Approved Procedures
The HACCP logs are being dry logged and they do not go back 90 days. Maintain logs daily. The sushi rice lab test expired in September. Provide an updated lab test. The HACCP plan could not be located. Submit updated HACCP plan and keep a copy on site for review. Employees could not operate the pH meter. Properly train staff in sushi rice testing and make sure the meter is working or rice cannot be served. There are no parasite destruction letters for fish. Provide.

Improper Maintenance of Walls/Ceilings
The hood vents are past their expiration date. Have the vents cleaned by a company approved by the City of Boston Fire Department www.cityofboston.gov/fire 2/6/2013 The hood was cleaned but the company is using the wrong stickers and is not on the fire department list. Will reffer to fire department.
Why This Matters
OVERHEAD CONTAMINATION: Peeling paint and debris fall into food. Condensation on ceilings drips bacteria onto prep surfaces. Studies found ceiling tiles harbor Bacillus cereus spores. Restaurant ceiling collapse contaminated food service for 200 guests.
Code Requirements
Walls/ceilings must be: Smooth and washable in food areas; Light-colored for easy soil detection; Free of holes, cracks, peeling paint; Sealed at all joints and attachments; Clean and free of grease, dust, cobwebs; Non-absorbent in areas subject to moisture.
Corrective Actions
Repair all holes within 14 days; Repaint with washable paint; Replace damaged ceiling tiles; Seal all penetrations; Clean walls and ceilings monthly; Fix any leaks immediately; Install FRP panels in splash zones
