Observed a leak at the bottom of the pipe located in the basement. (photos taken) No smell coming from the pipe but when the PIC flushed the toilet small amount of water came out of the pipe. PIC stated they had someone come out a month ago to fix the other side of the pipe. Exterminator also noted this on the recent report. PIC called the landload at the time of the inspection and asked to have a licensed plumber come out. Provide an invoice from a licensed plumber on company letterhead. PIC stated the licensed plumber is scheduled to come out today. Please email over the plumbers report after the repair has been fixed.

Controlling Pests (Pf)
KITCHEN-Observed rodent droppings. BASEMENT- a good amount of cockroaches on the glue boards. PIC was able to provide an updated pest report from 8/29/24. Spoke with the PIC and recommended to her that they make sure that they are reading the extermination reports and taking action to the recommendations that are stated on the report to help correct the rodent activity. Contact the licensed exterminator to come out and inspect the establishment. Provide a detailed IPM report for the next inspection. Clean to remove all rodent and cockroaches.

SEVERE CONTAMINATION: Rodent droppings carry 35+ diseases including Hantavirus (36% mortality rate), Salmonella (20% of cases from rodents), Leptospirosis (kidney/liver failure). Mice contaminate 10 times more food than they eat through urine and droppings.
