Observed a leak at the bottom of the pipe located in the basement. (photos taken) No smell coming from the pipe but when the PIC flushed the toilet small amount of water came out of the pipe. PIC stated they had someone come out a month ago to fix the other side of the pipe. Exterminator also noted this on the recent report. PIC called the landload at the time of the inspection and asked to have a licensed plumber come out. Provide an invoice from a licensed plumber on company letterhead. PIC stated the licensed plumber is scheduled to come out today. Please email over the plumbers report after the repair has been fixed. 9/13/24- PIC emailed on 9/11 stating that a plumber came out and stated the pipe needed to be replace because is was to old and that they patched the hole. Today I observed the pipe still leaking a small amount at the bottom. Photo taken. No smell. PIC was not able to provide an invoice from a licensed plumber stated the plumber would be out on Monday. She was not able to provide a confirmation email from the plumber. A woman from the building management stopped by and stated she would reach out to the plumber and have them send a confirmation email. The email that I recieved was from the building management company stating that a plumber would be out tomorrow at 7am. Please provide a report/invoice from the licensed plumber on company letterhead. Please ensure that a plumbing permit is pulled if this is required.

Controlling Pests (Pf)

KITCHEN-Observed rodent droppings. BASEMENT- a good amount of cockroaches on the glue boards. PIC was able to provide an updated pest report from 8/29/24. Spoke with the PIC and recommended to her that they make sure that they are reading the extermination reports and taking action to the recommendations that are stated on the report to help correct the rodent activity. Contact the licensed exterminator to come out and inspect the establishment. Provide a detailed IPM report for the next inspection. Clean to remove all rodent and cockroaches. 9/13/24- PIC stated that she called the exterminator and stated he would be out in 2 weeks. An email was sent to the PIC stating all the documentation that was needed for today. PIC was not able to provide confirmation on the extermination report. Please contact the licensed exterminator and provide a detailed IPM Report.
